Cityblock and Centene Form Primary Care Partnership in Florida

Medicaid value-based primary care company Cityblock is teaming up with Florida-based Sunshine Health, a subsidiary of Centene, to provide primary care and care coordination services to high-need and hard-to-reach Medicaid members. access in 11 Central Florida counties.

The partnership, which began this month, includes comprehensive services offered to members 24/7 and in a variety of modalities, allowing members the option to consult with their multidisciplinary care team virtually, at home or at a local clinic.

The partnership builds on Centene’s existing relationship with Cityblock in two states, New York and Ohio.

WHAT IS THE IMPACT?

Cityblock’s care model was designed for members with complex needs, and the company said many are at a disproportionately higher risk of health problems.

The care team aims to reduce unnecessary emergency room visits and avoidable hospitalizations while improving the quality of care and member engagement.

At launch, the partnership will serve members in Volusia, Seminole, Orange, Osceola, Polk, Hernando, Pasco, Hillsborough, Pinellas, Manatee and Sarasota counties in Florida.

“Cityblock has seen incredible growth in recent years, increasing our operations in our current and new states,” said Dr. Toyin Ajayi, co-founder and CEO of Cityblock. “We are pleased to launch into a new market with a strong Florida leader like Sunshine Health and to expand our national partnership with Centene. “

THE BIGGEST TREND

The partnership is Cityblock’s second in as many months. In April, the company revealed a partnership with Senior Whole Health by Molina to improve care for its members in Massachusetts to provide a team of healthcare professionals that will link Senior Whole Health members to personalized medical, behavioral health and social care support.

The focus will be on Molina’s dual-eligible members, who will be able to receive care from primary care providers, behavioral health providers and community health partners.

In 2023 Centene recorded profits of 2.7 billion dollars and revenues of 154 billion dollarsboth improvements year over year – 2022 saw profits of just $1.2 billion and revenues of $143.2 billion.
